Ettore Di Giacinto 40ec4ffc94 chore(openai): delete obsolete node-header helper and tests ...

The maybeSetNodeHeader / resolveNodeID helper and the chan-based
streaming test are fully replaced by middleware.ExposeNodeHeader and
its Ginkgo specs. Remove the old files. Update --expose-node-header
help text, ApplicationConfig.ExposeNodeHeader godoc, and the
distributed-mode docs entry to mention that the header also covers the
Anthropic and Ollama shims, and to reflect the new wrapper-based
implementation.
