Török Edvin a4e6ac4d78 remove "all domain scan" feature from phishcheck (--no-phishing-restrictedscan). Nobody is using it. ...

Don't care why an url is clean, just state it is clean.
Various cleanups resulting from this.
Prepare to introduce selective turn on of sub-features.
