* refactor(repository): ensure we always parse content.ID and object.ID
This changes the types to be incompatible with string to prevent direct
conversion to and from string.
This has the additional benefit of reducing number of memory allocations
and bytes for all IDs.
content.ID went from 2 allocations to 1:
typical case 32 characters + 16 bytes per-string overhead
worst-case 65 characters + 16 bytes per-string overhead
now: 34 bytes
object.ID went from 2 allocations to 1:
typical case 32 characters + 16 bytes per-string overhead
worst-case 65 characters + 16 bytes per-string overhead
now: 36 bytes
* move index.{ID,IDRange} methods to separate files

cli: major refactoring of how CLI commands are registered
The goal is to eliminate flags as global variables to allow for better
testing. Each command and subcommand and most sets of flags are now
their own struct with 'setup()' methods that attached the flags or
subcommand to the provided parent.
This change is 94.3% mechanical, but is fully organic and hand-made.

Both source and destination can be specified using user@host,
@host or user@host:/path where destination values override the
corresponding parts of the source, so both targeted
and mass copying is supported.

This allows using `EnableFileExpansion` to disable treating
arguments prefixed with "@" as file includes.
They now uniformly support 3 flags:
--prefix=P selects contents with the specified prefix
--prefixed selects contents with ANY prefix
--non-prefixed selects non-prefixed contents
Also changed content manager iteration API to support ranges.
