Whole code base: various cleanups, removed SINGLE_SOURCE_COMPILE relicts

* cleaned up code and improved coding style
* removed old SINGLE_SOURCE_COMPILE macro relicts
* use QString::toUtf8() instead of QString::toAscii() wherever possible
