Continue bootstrap-tables integration (#293)

Refactor manage_tables code and use proper js namespacing
Add glyphicons for most buttons in customers sections
Tidy up HTML in customers table page
Remove jquery-color plugin (not needed anymore)
Fix some minor css issues (feedback bar bottom margin)
