Jamie Pine 77aa26b7af Fix the thumbnail pipeline and decouple metadata backfill ...

Image thumbs no longer sit behind ffmpeg gates, sidecar versions bust the cache,
the sidecar layout moves to a shared crate, and metadata backfill is separated
from generation. Video dimensions are read for real; the cas_id tier and dead
settings are removed.
