The provider picker now draws the slot's health notice (the circuit breaker
is open) with the shared Notice component. When the selected add-on serves
the slot and declares the forget role, it also offers "Forget everything from
this provider", behind a confirmation dialog that says whether the change can
be undone. The action calls DELETE /api/chats/memory; the built-in provider,
Off and a provider that is down have none.
